Immunization Requirements for the 2020-2021 School Year

All students entering Kindergarten, Grade 7, or who are new to Montgomery County Public Schools (MCPS), must provide documentation of vaccinations. You may submit your child's vaccination record using a computer generated print out from the doctor's office, a vaccination booklet or submitting the Vaccine Health Form 896. Your child's vaccine record may be emailed to wresoffice@mcpsmd.org or dropped off at the school office on Tuesdays and Thursdays between 12:00 and 4:00 PM.
