Additional Documents
The following documents below must be provided at the time of registration as an attachment to the online registration. All attachments must be readable.
- Proof of Student Age and Identity - (A birth certificate or passport)
- New Student Information - Form 560-24
- Student Emergency Information - Form 565-1
- Immunization Certificate MDH - Form 896
- Dental Health - Form 527-17 (Only for incoming Kindergarten students)
- Proof of Identity of Parent/Guardian Enrolling Student and Relationship to Student (proof of Custody/Guardianship)- A photo ID such as a driver’s license or passport.
- Proof of Residency –
- If you own your home you must provide either the closing from the settlement OR a copy of your property tax bill.
- If you are renting you must provide a copy of the lease. If the lease is > 1yr, provide 2 pieces of mail with your name and address on them.
- If you are living with a family member or friend, please contact the school for the shared housing process.
