Quick Guide to Enrollment

All new to MCPS first-fifth grade students will need to complete:

1. New Student Enrollment Form (Spanish)
2. Student Emergency Form (Spanish)
3. Provide verification documentation:
   • Birth Certification
   • Proof of Residency
   • Parent ID
   • Student Grade Report
   • Student Immunization Records
4. Return all completed forms to the South Lake Elementary front office or send electronically to:
   • Ms. Cruz Larios at ingrid_a_cruzlarios@mcpsmd.org
   • Ms. Casteneda at karina_r_castaneda@mcpsmd.org

MCPS Enrollment Assistance:

1. For questions about enrolling in prekindergarten or Head Start, call 240-740-4530 and leave a message. A staff member will call you back to assist in filling out the forms.
2. For questions about enrolling international students in Grades 1-12, call 240-740-4500.
3. For all other questions, contact your school directly. You can determine the school assigned to your address by using the School Assignment Locator. A list of all MCPS schools can be found here.
