Student Emergency Information Forms

A message from KINGSVIEW MIDDLE SCHOOL

Dear Kingsview Parents/Guardians,

The first two days of school have been great!  During homeroom, your child should have received the Student Emergency Information Form 565-1.  You should have also received instructions on how to update your child’s emergency information online.  To update your child’s emergency information, parents/guardians will need to create a Parent VUE account using the Access Codes found on the letter mailed to the student’s primary address. Please take a few minutes this evening to review the revised instructions. 

If you have misplaced the letter or if you have forgotten your username and password, please contact Mr. Jason Godfroy, media specialist, at Jason_C_Godfroy@mcpsmd.org or call the Kingsview main office at 240-740-7130.

Please update emergency information in Parent VUE at your earliest convenience, but NO LATER than Sunday, September 12, 2021.  Please complete the bottom portion of the attached instruction sheet, sign it, and return it to your child’s period 1 teacher no later than Monday, September 13, 2021.  If there are any changes in student emergency information throughout the school year, it is extremely important that you make the main office staff know immediately to ensure that school staff have accurate information in the event of an illness, injury, or other emergency.
