  Required Form

The following form must be completed for all students enrolling in Montgomery County Public Schools (MCPS):

• New Student Information Form

  Residency Form

All students enrolling in MCPS must provide proof of residency (current property tax bill or lease). Refer to the following forms if applicable:

• Shared Housing Disclosure
• Determination of Residency and Tuition Status Form
• Request to Enroll Nonresident Student Form
• Homeless Status Form

  Immunization Form

All students enrolling in MCPS must show evidence of age appropriate immunizations. This form may be used as proof of immunization compliance:

• Maryland Department of Health Immunization Certificate
